Permission and activity risks
I am the player’s parent or legal guardian and have authority to give permission for participation. I voluntarily permit the player to participate in the activities described above. I will explain the safety rules to the player, provide appropriate footwear and shin guards, and tell the coach about any concern affecting safe participation.
Soccer involves running, changes of direction, physical exertion, balls and equipment, falls, and contact with players or surfaces. Risks include cuts, bruises, sprains, fractures, concussion, other head or spinal injuries, illness, heat-related problems, and, rarely, permanent disability or death. Coaching and safety precautions cannot eliminate all risks. I understand these risks in deciding whether to permit participation. This acknowledgment does not waive the player’s legal claims.
Health information and safe participation
I will disclose relevant allergies, medical conditions, injuries, limitations, and emergency instructions before training and update them promptly. The player will not participate contrary to a healthcare professional’s restrictions. The player must stop and tell the coach about pain, dizziness, suspected concussion, or feeling unwell. Soccer Seeds may pause participation and request appropriate medical clearance before return.
