Soccer Seeds Waiver & Release of Liability

I hereby release Soccer Seeds LLC, its agents, employees, staff, coaches, directors, officers, volunteers, and facility partners from any and all claims, responsibilities, or liabilities for injuries or harm incurred as a result of my (or my child’s) participation in Soccer Seeds programs.

1. INDEMNITY

I HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS Soccer Seeds LLC and all RELEASEES from any and all losses, liabilities, claims, demands, injuries (including death), or damages, including court costs and attorney’s fees, which may occur to myself, other participants, or my property arising out of my participation in Soccer Seeds activities — whether caused by my own negligence or the negligence or misconduct of others, in whole or in part.

2. PARTICIPANT’S RESPONSIBILITY

I understand the nature of the activities offered and affirm that I (or my child) am qualified, in proper physical condition, and willing to participate. If at any time I believe conditions are unsafe or my condition is not suitable for participation, I will discontinue involvement.

3. ACKNOWLEDGMENT OF RISK

I fully understand and accept that:

• (a) These activities involve RISKS of serious injury;

• (b) Risks may result from my own actions, the actions of others, or conditions of the facility;

• (c) Other unforeseen risks and social/economic losses may arise.

I FREELY ACCEPT AND ASSUME FULL RESPONSIBILITY for all such risks, losses, and damages.

4. MEDICAL AUTHORIZATION

I authorize Soccer Seeds LLC, its representatives, coaches, and medical providers to administer or obtain emergency medical care as deemed necessary, and I agree to be financially responsible for any such care or transport required.

5. EMERGENCY RESPONSE & MEDIA RELEASE

I hereby grant Soccer Seeds LLC, its agents, employees, staff members, directors, and officers permission to use my and/or my child’s name, picture, or likeness in any printed media, video, website, or any form of advertisement or promotional material. I fully release and discharge Soccer Seeds LLC from any claims or liability related to the use of such material. I waive all rights to any compensation or royalties arising from use of the media. I understand that I may be added to the Soccer Seeds email distribution list and can unsubscribe at any time.

6. ONGOING AGREEMENT

This agreement will remain in effect for any and all future participation at Soccer Seeds facilities, clinics, camps, training, or events.
